3.1 Notification

In the event of an accident notify immediately the OH&S officer/First Aid officer or the building warden for the building your in; after hours, if an undergraduate practical class, the lecturer in-charge of the class must be notified; for other cases outside 'normal hours', the security officer on 8946 6500.

3.2 Injury/exposure report

Anyone sustaining an injury or exposure that requires any kind of treatment, or which may require time off work, should complete an Injury/Accident Report form. These forms are available from the laboratory manager, the CTO or faculty office. A copy of the report will be filed with the SHARM Office.

3.4 Hazard report

A form is available (Appendix G) from the laboratory manager, CTO or faculty office for the reporting of potential hazards. Once completed by the individual expressing the concern, the form should be passed on to the immediate supervisor of the person for comment. When this is done, the form is submitted to the CTO for action. A copy of the report will be filed with the SHARM Office.

3.5 Unsafe condition report

If you think that any of the current safety procedures could be improved or that there is something dangerous, you should report the matter to your member of the OH&S committee, who will take the matter to the OH&S committee.
