New staff - Information on computer accounts

When receiving a new staff member there are some things that can be done to speed the process. The Staff account access form (.doc) provides a guideline on what you can do. ITMS will accept forms completed before your new staff member commences employment. Any signatures that we require from them will be obtained when they pickup their new account details.

Access to central IT services

Leaving CDU

Staff members ceasing employment with the university should complete this form to ensure their computer accounts are disabled upon leaving.

Phone and data billing request

Itemised extensions requests can be made with a fee of $20 per request regardless of number of extensions. This is also applicable to Charles Darwin University Students' Union, Schuberts Catering, etc.

Phone billing authority

When completing this form, note that check boxes are to indicate whether it is faculty or school information, group/branch or section information.

Complete the actual name of the area and cost code you wish to receive information for. If you are in a position that may require both, please indicate. (e.g. PA's to directors/deans require school/section information 'regularly', but faculty/group information 'on request only'). In completing the form, and forwarding to your manager (if appropriate) for approval, you are also signing the declaration. Your approver can then forward your email direct to Phonebilling@cdu.edu.au. For this purpose the email identification will indicate the person's signature.

Telephone and voicemail access

Complete this form to make changes to voicemail details for extensions. This form is required for the following:

NB: Include a cost code when requesting new numbers, conference calls, phone movement or additions - the job will not be passed to the contractor until a cost code is supplied.

Temporary account creation

For requesting a Temporary student account (.doc). This form covers creating a student account required for temporary use or for lecturers to use.

Under 18 approval form

Students: If you are studying at the university and under 18 years of age, you will need to provide formal approval of a parent or guardian before your account can be created.

Staff: If you are an academic staff member with a student-type teaching account which is used by students under the age of 18, the responsibility for proper use of such an account falls to you individually.
