Student forms
Complete this form is you are a staff member requiring teaching accounts in the student computer laboratories, a student requiring account creation 'exemptions' (i.e. exemptions from fee and enrolment checks in the case of enrolment delays), or require a temporary account in the student computer laboratories.
Students: If you are studying at the university and under 18 years of age, you will need to provide formal approval of a parent or guardian before your account can be created.
Staff: If you are an academic staff member with a student-type teaching account which is used by students under the age of 18, the responsibility for proper use of such an account falls to you individually.
Send all forms to ITMS Client Support, Red 3, Casuarina campus.
