Closing the Gap – improving integrated care through a clinical decision support tool’

Abstract
Kidney disease in Australia disproportionally affects remote living Indigenous Australians, who experience end-stage kidney disease (ESKD) six times that of non-Indigenous Australians. Integrated systems of care which focus on prevention and care coordination can slow the progression to ESKD and reduce complications of inter-related illnesses such as diabetes and cardiovascular disease. In the NT, where the Aboriginal population is highly mobile and often access multiple community and hospital-based services, there are significant challenges to the provision of optimal integrated care.

Biography
Gillian’s background includes more than 20 years’ experience in the health and policy environment in the NT. She moved to Menzies in 2013 to oversee the development of the renal program. Her interest is in the delivery of projects that focus on evidence translation and she is currently completing a PhD in the health economics of different dialysis models of care. Gillian currently leads the development and implementation of Territory Kidney Care, an integrated clinical information and decision support tool. Read more on our website.
