Abstract: The outback town of Katherine, Australia, serves 24,000 people, including 60% who identify as Indigenous. The homelessness rate is more than 30 times the national average.  Each year, over 600 homeless people present 5 or more times to the hospital emergency department, get patched up, then return to life on the streets.  Services in Katherine have united to support the most vulnerable to fulfil their health, cultural, spiritual, housing and family support needs. These social innovators have chosen to work with a developmental evaluator to co-create change, and transform into a person-centred network of services, assessing the needs of the frequent presenters, providing collaborative case management and working together to support participants. Menzies is working alongside the initiative to evaluate the Collective Impact process and the outcome for participants.
