  • The direct Admissions Person form (ADMF3000), only when the Health Provider button (middle block) is selected (refer ADMF2M30)
  • The Record Enrolment form - Person screen, only when the Health Provider button is selected (refer ENRF0192)

  • The Start Date and End Date of the Health Care cover
  • The code of the Health Care Provider
  • The Type of Health Care cover
  • The Health Care membership or policy Number
