This form is used to record and maintain the institution-defined set of Disability Type Codes. These are mapped to Government Disability Codes (element 615), but provide for greater flexibility with the ability to subdivide Government Codes and to use more meaningful codes. Each institution-defined Disability Type Code must be mapped to a System-defined codes which match the Government Disability Codes for element 615. Disabilities for a student are recorded in ENRF3040 or Callista Connect. Disability data is reported to the government in the Disability extension of the HE Student and VSL Student data groups.

To create a new institution defined Disability Type using the Maintain Disability Types form:

• Enter Insert mode
• Enter the new Disability Type Code in the Disability Type field of a blank record
• Enter the corresponding description for the Disability Type in the Description field
• Select the System Disability Type from the list of values
• Record an explanation for each Disability Type. Disability Type explanations are used as part of the GOVT-STATS application in Callista Connect. The explanation is used as an answer to the Disability Type question in the application
• Save

Rules/Notes:

More than one active institution defined Disability Type can exist for each Sytem Disability Type.

Institution defined Disability Codes MUST be unique. 

Explanations should be recorded in the form of an answer for example:

• I have a hearing impairment
• I have a medical impairment

If an explanation is not recorded, the radio button appears for selection in the GOVT-STATS application with no explanation text next to it.

• Find the record to be modified
• Modify the record as required
• Save

Rules/Notes:

Once in use, an institution defined Disability Type should not be modified.

To delete an institution defined Disability Type using the Maintain Disability Types form:

• Find the record to be deleted
• Locate the cursor in the record to be deleted
• Delete record
• Save

Rules/Notes:

An institution defined Disability Type cannot be deleted if it has been used in a person disability record. It can be closed preventing any further use by selecting the Closed check box.
