Aged Care Transition to Practice Program

About the program

The Aged Care Transition to Practice (ACTTP) program is a fee-free program, and is designed for nurses transitioning to aged-care specialty practice within the last two years. This grant-supported initiative builds the confidence, knowledge and skills to advance culturally informed, person-centred care of the older person in various care settings and contexts.

What you will learn

This program will empower participants to develop the knowledge and skills to provide culturally informed person-centred care that promotes nursing care through supporting personhood; respecting the older person’s rights, strengths and dignity; and promotes the person’s health and well-being and supports participatory engagement in decisions on care and treatment.

An individualised learning plan will be developed in consultation with all participants.

Eligibility

Participants eligible for this program:

  • Enrolled Nurses and Registered Nurses registered with the Australian Health Practitioner Regulation Agency (Ahpra).
  • Nurses must be employed in aged care, such as in a residential or home care setting, or in a hospital system with older people in gerontology sub-acute care settings
  • Newly graduated enrolled nurses and registered nurses within two years of practice or have recently transitioned to aged care or care of the older person settings
  • Nurse practitioners within two years of practice in aged care or care of the older person settings
  • First Nations (Aboriginal and/or Torres Strait Islander) nurses are encouraged to apply.

Program structure

This program includes regular support from an academic lecturer and mentorship support from a mentor working in aged care. The program is delivered in a hybrid mode that is completed over 10 months, which includes:

  • 7 core self-paced online learning modules
  • 3 elective self-paced online learning modules
  • 7 live webinars
  • 2-day interactive workshop

Program participants may select the workshop offered at any one of Charles Darwin University campuses in Alice Springs, Brisbane, Darwin, Melbourne, Perth or Sydney, depending on their convenience.

This program received grant funding from the Australian Government. Fee-free places are available. Contact us to find out more.

Credit transfers and advanced standing 

Completion of the program will result in a total of 100 hours of continuing professional development over 10 months. Registered nurses who complete the ACTTP program will be eligible to apply for recognition of prior learning for a unit of study in the Graduate Certificate of Older Person Nursing Care.

This is a non-credit bearing program and does not form part of Charles Darwin University's accredited course offerings.

Learning materials

All learning materials are included - videos, readings, case studies, and quizzes are provided via the online platform.

No additional textbooks or purchases are required. Participants are encouraged to fully engage with the provided resources to support the successful completion of each learning topic.

Study and career pathways

Upon completion of this program, Registered Nurses will be able to enrol in either the Graduate Certificate of Older Person Nursing Care or the Master of Clinical Nursing.

Want to be a mentor? 

Mentors provide educational and professional development support for the participant's individualised learning plan. Mentors have the option to enrol in the free short course 'Mentor Program' offered by Charles Darwin University to further develop their mentor skills. Mentors will be invited to be part of a Community of Practice for mentors for support, feedback sessions and professional development opportunities.   Mentors will receive a Certificate of Achievement at the end of their mentor program.

  • Mentors must be employed in aged care for a minimum of two years, such as in a residential or home care setting, or in a hospital system with older people in gerontology sub-acute care settings
